Proof of Covid-19 Vaccine for Writing Classes and Delves

Literary Arts requires proof of vaccination or a negative PCR test (within 72 hours) for our in-person classes and events. All students or attendees will be asked to show proof of full vaccination (vaccination card or photo of vaccination card) before the first day of class/upon entry to the event OR proof of a negative COVID test taken within the last 72 hours before each class session/upon entry to the event. At least 48 hours before the start of your first in-person writing class or Delve seminar, please send us proof of vaccination or negative PCR test via this form. Valid forms of proof include a picture of your vaccination card or a PDF of your vaccination card or negative PCR test. This form is secure and records will only be seen by Literary Arts staff to verify that all attendees are vaccinated or have a negative PCR test. While Literary Arts is taking precautions and enforcing safety measures, we cannot guarantee zero risk; a risk of exposure to COVID-19 exists in any public setting. By entering the classroom or performance venue, students, teachers, and attendees accept the risk of exposure and knowingly waive and release Literary Arts from any liability related to COVID-19.
